Aim of the lecture is to highlight how important is that the clinician knows all the possible anatomical variations to prevent missed anatomy and to get the positive outcome of the therapy and how the operator can make a rational and synergistic use of 2D and 3D imaging in order to provide the best service to the patient with the least possible biological cost.
Part II: how to respect the root canal anatomy during the shaping
The first goal is to underline how rotary NiTi files can create alterations that may result in a failure of the endodontic therapy if misused. The second goal is to highlight how nowadays in nonsurgical endodontics, thanks to the new system, a totally mechanical shaping can be safely performed by the clinician, getting high quality and original anatomy respectful shaping, even in complex anatomies and even using a reduced number of instruments.

The people are living longer and want to mantain their natural dentition.
There are several factors that might influence the development of varying degrees of moderate and severe calcification of the pulp chamber as well as the root canal system, like, multiple restorations, trauma, vital pulp therapy and chronic irritation from deep restorations or cracks.
Pulp canal obliteration, also called calcific metamorphosis, is a sequelae of tooth trauma, and it’s generally asymptomatic and clinically the patients present to the practitioner with yellow discoloration of the affected tooth crown and apparent loss of the pulp space radiographically.
The critical management decision is: when to treat these teeth endodontically ? The difficulty degree of the clinical management of this kind of cases, the practicioner should be aware of the possible complications, that may occur.
This lecture tries to give some tips to help the clinician to deal with this clinical challenge.
