Etiological factors of worn dentition
Tooth wear represents a frequent pathology with multifactorial origins. Behavioural changes, unbalanced diet, various medical conditions and medications inducing acid regurgitation or influencing saliva composition and flow rate, trigger tooth erosion. Awake and sleep bruxism, which nowadays are widespread functional disorders, induce attrition. It then became increasingly important to diagnose early signs of tooth wear so that proper preventive, and if needed, interceptive-restorative measures are taken. Such disorders have biological, functional and also aesthetic consequences.
Modern methods of treating worn dentition
Following a comprehensive clinical evaluation, treatment objectives, such as a proper occlusal and anatomical scheme as well as a pleasing smile line, are usually set on models with an anterior teeth or full mouth waxup, depending on the severity of tissue loss. Based on the new Vertical Dimension of Occlusion (VDO) set on the articulator, combinations of direct, semi-direct and indirect restorations (using composite resins or monolithic high-strength ceramics) can help to re-establish anatomy and function. The use of adhesive and ultra-conservative techniques has proven its potential, in particular for the treatment of moderate to advanced tooth wear. A modern approach to treat tooth wear shall involve a specific and detailed treatment planning involving its aetiology, risk factors control and the biomechanical status of each tooth to be treated.

The value of the natural tooth has diminished dramatically among many dentists, who no longer see themselves as a dentition-preserving healthcare provider but rather prefer to work with screw and nuts. Even minor concerns about the prognosis of a tooth needing endodontic treatment often lead to extraction followed by implant replacement. This action, under the best circumstances,is probably due to poor knowledge of the true survival rate of endodontically treated teeth but in the worst scenario it has to be considered just business! If a tooth is deemed restorable from a restorative and periodontal aspect, endodontic therapy should be the first treatment option considered. If a tooth has a poor restorative or periodontal prognosis, extraction and implant should be considered.
It is important to note that the two treatment alternatives have different aims; endodontic and restorative treatments are provided to treat or prevent apical periodontitis and to give back function, whereas implants are used to replace missing teeth.
Nowadays the maintenance of “a nice smiling” represents the main desire of our patients and the request for tooth coloured restorations is very high in posterior areas too. Predictable and repeatable results can be achieved through materials and clinical practices that, on one hand, preserve the original structure of the compromised tooth and, on the other hand, involve a very low biologic price. New technologies for the enamel-dentin adhesion and the continuous evolution of more sophisticated esthetic materials (composite resins and porcelains) allow the effective reaching of verifiable and predictable results.
Technologies and materials have to follow an operative approach that strictly involves operative steps which have been carefully codified referring to the materials typology used from time to time.
This lecture aims to point out the relevant guidelines for indirect posterior esthetic restorations for both vital and not-vital teeth in order to reach a correct.standardized and longlasting clinical result.

Tooth preparations without a defined finish line have been termed in several different ways, such as a knife edge, feather edge, or shoulderless. With slight differences between each other, all this preparation types may be defined “vertical” as opposed to “horizontal” ones (shoulder, chamfers) and since the introduction of metal ceramic they have been almost abandoned.
- The distinction between clinical conditions where the existing occlusal relations can be left unchanged and clinical conditions where a reorganization of occlusal schemes may give restorative advantages and a better long term prognosis to the therapies performed.
