A particularly emerging topic will be addressed: peri-implant diseases. The non-surgical treatment has proven quite successful in case of mucositis. Vice versa, in case of progressive and irreversible bone loss, the non-surgical solution is not predictable, however, it is recommended as a first therapeutic phase. In some cases, clinically well documented, the non surgical option may have very satisfactory long term outcomes. In other cases can help to prolong significantly the survival rate of severely compromised implants.
It is essential that non-surgical treatment is all the time specific and tailored to the needs of each individual patient. The protocol, particularly composite and differentiated, associated with the use of diode laser, is described and illustrated with clinical images, drawings and videos.
The long-term success of any dental and peri-implant treatment, carried out in a proper manner, depends essentially on the patient’s degree of cooperation, both in adhering to the recommended recall schedule, and in daily proper home care.

Within traditional beliefs of peri-implantitis, plaque accumulation is the primary etiologic factor. A more modern approach looking beyond symptomology using stratification of peri-implantitis cases into risk groups has elucidated that, in most of cases, bacterial superinfection seems to be secondary. More important factors for triggering peri-implant disease seem to be deficiencies in implant placement or their prosthetic restoration. Knowledge of these contributing factors will substantially change not only the treatment plan, but also the prognosis of peri-implant disease. The aim of this presentation is to demonstrate the do’s and don’ts of good clinical practice to avoid peri-implant pathology and obtain a longitudinal stable clinical response of the peri-implant tissues.
Following this presentation, participants will be able to:
1) describe different etiology and risk groups of peri-implant pathology,
2) develop an individualized treatment plan addressing specific risks for peri-implantitis
3) understand the role and efficacy of surgical approaches to peri-implantitis.

The surface of dental implants determines the initial phases of the biological response to the implant and affects its ability to integrate into the surrounding tissue.
In four consecutive studies since 2007 more than 300 different, sterile-packaged dental implants were analysed by scanning electron microscopy and elemental analysis (EDS). Nearly 50% of all analysed implants showed a wide variety of organic and/or inorganic impurities containing iron, copper, tin, chromium, tungsten, antimony and nickel. Additional Time-of-Flight Secondary Ion Mass Spectrometry (ToF-SIMS) analysis revealed polysiloxanes, polyethylene and even hazardous surfactants on sterile dental implants.
